ELIGIBILITY AND REFERRALS
Students must meet the following eligibility and low vision criteria in order to attend a CLVP-BC clinic. Please request a referring professional to submit an application if your child would benefit from attending one of our clinics.
1. Confirm Eligibility
To be eligible for the CLVP program, a child must:
- Be aged 4-19
- Have a qualifying visual impairment (see below)
- Be enrolled in preschool through Grade 12
- Have a valid BC CARE card
2. Confirm Low Vision Criteria
Visually impaired students must meet one or more of the BC Ministry of Education Visually Impaired criteria:
- A visual acuity of 6/21 (20/70) or less in the better eye after correction
- A visual field of 20 degrees or less
- Any progressive eye disease with a prognosis of becoming one of the above in the next few years
- A visual problem or related visual stamina that is not correctable and that results in the student functioning as if his or her visual acuity is limited to 6/21 (20/70) or less
3. Obtain a Referral
Who can refer a student?
Learners with visual impairments can be referred by the Sunnyhill Hospital Visual Impairment Program, a Teacher of the Visually Impaired, an Ophthalmologist or Optometrist.
A CLVP Referral / Screening Form MUST be completed and submitted for approval. Referring professional (vision teacher, doctor) will be contacted by CLVP to confirm your child's spot in an upcoming clinic.
Parents/Guardians: Please do not complete and submit any forms until you have received confirmation of your child's acceptance into a CLVP clinic from your Referring professional.

4. Complete Forms
Your child's vision teacher/eye doctor will contact you confirming acceptance into the CLVP clinic. Parents will be requested to complete the Parent, Consent and Video/Photo forms once your child's placement in a clinic has been confirmed. Please access these forms below.
All forms are due back to your child's vision teacher three weeks prior to the clinic date. Failure to return forms fully completed by the due date requested may result in your child's clinic spot being given to another student on the wait list. You will be reminded of the importance of returning the forms before this takes place.
Please ensure your child's clinic placement has been confirmed before you download and fill out the forms below.
