INFO FOR VISION TEACHERS

If you have a specific clinic appointment time request, please email Stacey Ashton-Shefsky (stacey.ashton-shefs@sd23.bc.ca). Appointment time slots are set on a first-come first-served basis. Please specify on the Educator Form if you prefer an AM or PM appointment and, if the clinic is over two days, which day.

Please ensure that all fields are completed on all forms before submitting. Some fields may be repetitive on the forms, but these forms are reviewed by different CLVP team members prior to the clinic.

In lieu of the Medical / Doctor Form, a recent eye report can be sent. Either of these documents are vital in the team preparation for the clinic.

Regional vision teachers must collaborate and prioritize their clinic referral / screening forms where there are more requests than clinic spots available.

Vision teachers and parents should be prepared to stay for the entire clinic appointment. Appointments can take up to 2 - 3 hours per student.

There will be some snacks available, however, vision teachers and families are responsible for their own lunches.

If the child is seen by another doctor or specialist that we have referred them to, please ensure that a copy of that report is sent to the CLVP student file.
